An Offaly care centre for adults with disabilities has been found non-compliant in a number of areas by HIQA.
The healthcare body issued a number of 'urgent action' orders to Saint Anne's Residential Services, ran by Avista Limited, following its inspection in April earlier this year.
St Anne's is a single story bungalow centre in a rural part of Offaly, which can care for a maximum of four adults with an intellectual disabiliity.
At the time of the unannounced inspection there were three people staying there.
Staffing levels were found to be inadequate, while those working there lacked the required training to deliver a safe quality service.
Over a five day period this year, four different agency staff, each new to the centre, were employed to work shifts.
There were no records of agency staff being trained in fire safety, positive behaviour support, and epilepsy.
A lack of management staffing was also highlighted by HIQA.
The Person in Charge was responsible for two other centres, leaving the house manager to assume full responsibility for the day to day operation.
Despite the arrangement, the manager did not have protected time for administration duties and often had to work on the floor to meet staffing needs.
The centre also failed in the inspection in the regulation area of risk management.
HIQA says a satisfactory compliance plan was subsequently put in place outlining plans to address the issues.
