An interim report says there have been 40 allegations of abuse at the Portlaoise facility since 2023.
HIQA is referring their concerns over a Laois nursing home to the Gardaí.
The move will be confirmed by their Chief Executive Angela Fitzgerald when the healthcare watchdog appears before the Oireachtas Committee on Health this morning.
It follows an RTE Investigates report which documented multiple instances of abusive practices at The Residence in Portlaoise, and at Beneavin Manor in Dublin.
Both facilities are owned by Emeis Ireland.
The Health Information Quality Authority says they have issued warnings of cancellation of registration if the owners fail to act.
An interim HIQA report states there's been 40 notifications of allegations of abuse at The Residence Portlaoise since December 2023, while there's been 198 in Beneavin Manor.
