Over 40,000 cars failed the NCT inspection in the midlands last year.
New figures from the National Car Test Service show over 8,000 motorists had their vehicle labelled "fail dangerous."
Over half of vehicles that went through the NCT in Athlone last year failed, the highest rate in the region.
However, in the other three centres in the midlands, Mullingar, Portlaoise and Tullamore, more than 50% of inspections passed.
7% of inspections in the region resulted in a "fail dangerous."
Around the country, just under 750,000 vehicles failed the NCT, which is almost half of those checked.
According to CarTakeBack.ie, steering and suspension, lighting and electrical and the side slip test are the top three reasons for an NCT failure.
Porsche, Lexus and Land Rovers were among the most common brands to pass the inspection.
