It was first seized in 2009, and again last year.
A shotgun seized by Gardaí 15 years ago ended up back in the hands of criminals in the midlands.
The pump action shotgun was supposed to be in possession of Gardaí since it was seized back in 2009 in the east of the country.
However, as part of an operation into a feud between two criminal gangs last year, the gun was seized again.
A criminal investigation is now underway into how it left the control of Gardaí.
It's understood the Garda Commissioner Drew Harris has given a report to the Justice Minister and the Policing Authority.
