Three facilities for people with disabilities are non compliant with regulations following HIQA inspections this summer.
The Authority has published 21 reports, of which 18 centres have good standards of care.
However, concerns have been raised about a unit in Westmeath where staff said some residents refuse to leave the building during fire drills.
