The high number of adverse incidents in the health services is "not surprising".
That's according to the Industrial Relations Officer for Midlands Regional Hospitals in Tullamore and Portlaoise, Gráinne Walsh.
Figures show there were over 5,200 adverse incidents, which include incidents such as falls, surgery on the wrong patient or death, in the midlands last year.
The Irish Patients Association estimates there has been 174 preventable deaths in CHO8 in the past five years.
