11 food businesses were issued closure notices nationwide.
An Offaly business has been issued a closure order over human waste overflowing from a staff toilet.
The Village Shop &Takeaway in Mountbolus is one of 11 food businesses that were ordered to close last month, as human waste was found to have been overflowing into a room storing food contact equipment.
Elsewhere, the Food Safety Authority of Ireland inspectors found a dead rat near food, a cockroach infestation in a food preparation area, and a disused cooker being used as a bin.
The FSAI says the issues identified are deeply concerning and demonstrate serious failures in basic food safety controls.
The businesses are also located in Dublin, Tipperary, Wicklow and Kerry.
