A Midlands care centre is being cited by the health care watchdog for its failures in safeguarding and staffing.
The Muiríosa Foundation centre in Westmeath has been found non-compliant by HIQA in four of the five regulation areas it was inspected on.
The facility comprises of three locations all within close proximity to a small town in the county.
A two day unannounced inspection came after HIQA received solicited information detailing a number of safeguarding and protection concerns.
It found the centre did not have the appropriate number of staff on duty at all times to ensure people in the centre were safe and protected.
The admission of a resident which did not follow its own admissions policy had resulted in significant safeguarding concerns.
The report says oversight and management of admissions, safeguarding and risk was poor and directly impacted the lived experience of residents in one part of the centre.
Management of the premises was found to be substantially compliant with all three locations in a good state of repair and laid out to meet the assessed needs of the residents.
However it failed in the area of risk management procedures after a review of incident reports for one resident showed 19 distress behaviour incidents this year, with 16 of those in May alone.
Despite the increase in in distress behaviours, their assessment had not been reviewed since March.
Management at the centre says an emergency response is being applied to the staffing shortage, and a full review of its admissions policy and risk management plans has been carried out.
