The man, identified as “Mr. D,” died on August 2, 2021.
The Irish Penal Reform Trust is calling for a systemic review of security measures in prisons.
It follows a report into the death of a man in Midlands Prison in Portlaoise in 2021, which revealed staff were unable to locate the key to his cell while he was having an allergic reaction.
The Office of the Inspector of Prisons said a nurse 'watched helplessly' through the flap of the door, while the 59-year-old man lost consciousness and died.
The key was locked in a separate room, and the staff member responsible for it was on a break.
