A breach was recorded at a midlands breach.
The HSE recorded 553 data-protection breaches in the first nine months of this year.
According to freedom of information documents, they include medical files found in car parks and public footpaths.
Documents were also found in a patient's bag after they were discharged from Mullingar Regional Hospital.
