A Laois centre for people with assessed disabilities has failed a HIQA inspection in four regulation areas.
The Muiríosa Foundation Centre was the subject of an announced visit in May earlier this year.
This inspection was carried out to assess the facility's progress in implementing a compliance plan following a visit in October last year.
The report is also being considered as part of HIQA's decision on renewing the provider's registration.
While the inspector observed a lot of positive care and support practices, the centre was found non compliant in four areas.
It's governance and management failed the inspection over ineffective financial and fire safety audits.
The provider's management also failed to carry out an assessment of the mobility and transfer needs of a resident who experienced six falls over the course five months.
The person has a significant visual impairment, is a wheelchair user, and the inspector was informed that generally one staff supported them with all transfers and mobilisation.
There was no record in the centre that an assessment of their needs had been completed by a relevant allied health care professional, either before or after these falls.
As a consequence, the centre failed in the area of risk management.
It's also been found non-compliant on fire safety grounds and in the area of protection due to on-going problems with resident's finances.
An compliance plan is being put in place by the Person in Charge to address the issues highlighted.
