GeoView's quarterly report shows that one in three commercial properties in Edenderry remains empty.
The vacancy rate in the east Offaly town is 29% - pushing Ballybofey in Co. Donegal into second place by 2%.
Overall, the Faithful County also has the highest vacancy rate in the midlands at 15.5%.
Laois is next on the list with around 14% of retail properties vacant, an increase of 1% on last quarter.
Portarlington has the highest rate in the county, while Portlaoise has the lowest.
Westmeath fared out best in the final three months of 2019.
Just over one in ten retail properties are vacant there, making it the lowest in the region and below the national average of 13.3%.
Athlone has the highest vacancy rate in the Lake County at almost 20%, while Moate has the lowest at 13%.
In total, 16 counties recorded an increase in the last quarter, six rates decreased and four remain unchanged.
