Multiple issues highlighted included failure take reasonable measures to protect residents from abuse.
An Offaly nursing home has been found to be non-compliant in eight of nine areas during an inspection by the health watchdog earlier this year.
HIQA inspectors found Esker Ri, in Clara, failed to notify the Chief Inspector, within three working days, of an incident of suspected abuse of a resident.
The report, which is based on an unannounced visit to Esker Ri Nursing home in Clara last August, found staff were not appropriately trained to deliver safe care, and were not suitably supervised.
It also showed the management of records was also not in line with regulations, and there wasn't an effective management structure in place.
Inspectors also noted there was a failure to adopt and implement policies designed to support and protect residents, and the facility didn't ensure relevant information about those living there was provided to the receiving hospitals.
The report says the provider failed to take reasonable measures to protect residents from abuse.
However, residents said staff were kind, friendly, respectful, and made them feel safe in the centre.
In response, Esker Ri Nursing Home says the identified staff have been trained, a full audit has been carried out to ensure all personnel files are compliant, and a defined organisational structure has been put in place.
It also says all staff have been trained on safeguarding, and are fully aware of who they should report any suspected safeguarding issues to.
