Bryonny Sainsbury died after being crushed by a horse in August 2021.
The HSE says its "profoundly sorry" for a series of egregious errors that took place while a Midlands woman was in its care.
Bryonny Sainsbury died after she was crushed by a horse during veterinary procedure at a riding stable in Co Longford on 26 August 2021.
The 25 year old was admitted to the Regional Hospital in Mullingar where some of the advice issued by specialists at Beaumont Hospital was not followed.
A 2024 inquest found that she could have been saved if she was transferred to Beaumont sooner.
Ms Sainsbury died from her injuries a few days later on August 31st .
RTE is reporting hospital management offered a "sincere and unreserved apology" as the family settled a case against the hospital over the failures in care given.
