The FSAI raised five concerns with the restaurant.
An Offaly restaurant was among eight businesses issued a closure order in August.
Cafe India in Tullamore was ordered to close for over a week last month by the Food Safety Authority of Ireland.
In its latest report, the FSAI says the restaurant failed to comply with food legislation.
Inspectors found the dough mixer and food storage containers to be unclean, while workers demonstrated poor food safety knowledge when questioned.
They also noted written allergen information wasn't provided to customers for the traditional Indian desserts on the menu.
The report raised concerns with the storing of a number of food items in an external yard, where they weren't protected from the risk of contamination.
The FSAI lifted the restriction order on August 27th.
Seven other businesses were issued a closure order last month - they were in Dublin, Donegal, Limerick and Tipperary.
