An inspection was carried out unannounced earlier this year.
The Psychiatry Department of Roscommon University Hospital has received a 73% compliance rate following an inspection.
The approved centre has seen a decrease in compliance every year since 2022.
The Mental Health Commission conducted an unannounced inspection of the Department over the course of four days in May.
The centre was found to be compliant in 24 areas and non compliant in nine.
Areas of concern highlighted in the final report include the use of seclusion, consent to treatment and staffing.
It was found that staff could not clearly observe people in seclusion, and patients in those areas did not have ready access to sanitary facilities.
Capacity assessments were not carried out on patients prior to consent to treatment and no documentation was found of the patients' ability to understand the nature, purpose and likely effects of medications.
The unit was found to be complaint with general health and safety regulations and in the provision of people's general health.
Inspectors did note there were strong frameworks and structures evident in the centre.
At the time of inspection, there were 21 residents in the 24-bed facility, four of which had been there for longer than six months.
