The area which covers the midlands has seen a jump by 10% in adverse incidents from 2021 to 2022.
The Irish Patients Association is calling for transparency over the number of preventable deaths in Irish hospitals.
It comes as new HSE data shows adverse incidents in the CH08 area, which includes Westmeath, Laois and Offaly, jumped by 10% from 2021 to 2022.
The IPA estimate that 174 preventable deaths have occurred in CHO8 in the last five years.
Nationally, almost half a million adverse incidents were reported in the past five years, with over 3,000 deaths associated with those cases.
