Uisce Eireann is putting an action plan in place after waste water pollution in Westmeath.
The Environmental Protection Agency and Inland Fisheries Ireland were notified following an issue with the operation of storm water screens at the Mullingar Wastewater Treatment plant.
The assessment found flows to the storm water tank were partially restricted preventing full use of available storage during extensive rainfall.
The utility says immediate action is in place to prevent a reoccurrence.
