Five safety recommendations have been issued in the meantime.
Irish Rail is being issued with an Urgent Safety Notice by the Railway Accident Investigation Unit after a series of near misses with its own staff since 2023.
The notice document lists eight reported near misses with employees - nobody was injured in any of these incidents.
However the RAIU says a trend investigation is underway into near misses in Westmeath, Offaly, Carlow, Dublin and Wicklow.
It has made five safety recommendations in the interim period, including a call for a review of track inspection methods with a view to introducing technology to minimise the need for patrols by staff.
Irish Rail says a series of measures have already been initiated to improve safety, including moving track patrols to night time, the use of drones instead of employees to check infrastructure and 10 million euro invested in safe walkways and access points.
